1. Scope of Services

AF360 Recovery provides massage therapy and recovery services intended to support wellness, pain relief, muscle recovery, stress reduction, flexibility, and general physical comfort.

Massage therapy is not a substitute for medical care, diagnosis, or treatment. Therapists do not diagnose illness, prescribe medication, perform spinal adjustments, or treat medical conditions.

Clients should consult a physician or other qualified healthcare provider regarding any medical condition or concern.

2. Medical Conditions and Client Responsibility

Clients are responsible for providing complete and accurate information regarding:

• Medical conditions

• Injuries

• Surgeries

• Medications

• Allergies

• Pregnancy

• Areas of pain or discomfort

• Any condition that may affect the session

Clients agree to notify the therapist immediately if there are any changes in health, discomfort during the session, or concerns regarding treatment.

AF360 Recovery reserves the right to refuse or discontinue treatment if the therapist believes massage may be unsafe or inappropriate.

3. Results Disclaimer

Results from massage therapy vary from person to person. AF360 Recovery does not guarantee specific results, including relief from pain, improvement in range of motion, reduction of symptoms, or recovery from any injury or condition.

4. Consent to Treatment

By receiving massage therapy services, you voluntarily consent to treatment.

You understand that:

• You may stop or modify the session at any time

• You may request changes in pressure, technique, temperature, music, or positioning

• Certain areas of the body may be excluded at your request

• You are responsible for communicating discomfort or concerns during the session

Massage therapy is entirely therapeutic and non-sexual in nature.

Any inappropriate language, conduct, or behavior will result in the immediate termination of the session, and full payment will still be due.

5. Minors

Clients under the age of 18 must have written consent from a parent or legal guardian.

A parent or legal guardian may be required to remain present during the session.

6. Appointments

Appointments are subject to therapist availability.

AF360 Recovery reserves the right to reschedule or cancel an appointment due to illness, therapist availability, weather, emergencies, safety concerns, or circumstances beyond our control.

Late arrivals may result in a shortened session in order to avoid affecting the next scheduled appointment. Full payment for the originally scheduled appointment time will still apply.

7. Cancellation and No-Show Policy

Appointments may be canceled or rescheduled without charge with at least 24 hours' notice.

Appointments canceled with less than 24 hours' notice, same-day cancellations, and no-shows will be charged 100% of the scheduled service amount.

For memberships, packages, or prepaid sessions, the session may be forfeited.

Repeated no-shows or cancellations may result in refusal of future appointments.

8. Card on File Authorization

By booking an appointment, the client authorizes AF360 Recovery to maintain a valid debit or credit card on file.

The client authorizes AF360 Recovery to charge the card on file for:

• Late cancellations

• No-shows

• Unpaid balances

• Membership renewals

• Additional time or services requested

Clients are responsible for keeping card information current.

9. Memberships and Packages

Memberships and packages are non-refundable and non-transferable.

Unused sessions have no cash value.

Memberships automatically renew unless canceled at least 7 days prior to the next billing date.

AF360 Recovery reserves the right to modify membership pricing or benefits with reasonable notice.

10. Liability Waiver

Clients voluntarily assume all risks associated with massage therapy and recovery services.

To the fullest extent permitted by law, AF360 Recovery, Rest Network LLC, its owners, therapists, employees, and contractors are not liable for:

• Injuries or conditions not disclosed by the client

• Aggravation of pre-existing conditions

• Reactions to massage oils, lotions, or products

• Loss or damage to personal property

• Delays, interruptions, or inability to provide services

Nothing in these Terms limits liability where prohibited by law.

11. Payment

Payment is due at the time of service unless other arrangements have been made.

AF360 Recovery accepts debit cards, credit cards, HSA/FSA cards when applicable, and other approved payment methods.

Returned payments, chargebacks, or disputed charges may result in suspension of future services until resolved.
